There are some caveats to reckon with:
- The MP is only meant for OM12;
- You can’t install an Operations Manager Agent on a Service Management server (SM or DW) due to a technical incompatibility;
- Management of those servers must be done agentlessly for now.
The MP (in 11 different languages) can be downloaded from here. The English version is named: ‘System Center 2012 Monitoring Pack for Service Manager.msi’ and is to be found on the bottom of the whole list of MPs.

Like any other MP: RTFM is required, before importing the MP, while importing the MP and after the MP is imported.

Interestingly, my SCSM environment is reporting happily to my OpsMgr 2007 environment.
We have AD integration enabled in our environment, so once I deployed SCSM, the management service was updated with the OM management group detail and the SCSM servers appeared in pending management :)
Haven't run into any clashes so far.
